Use this form if:
- you have retired after your preservation age (currently age 56); or
- have reached age 65, even if you are still working.
If you wish to commence a pension whilst continuing to work under age 65, use the Application for Transition to Retirement Pension form.
You are eligible for a transition to retirement pension if:
- you have reached your preservation (currently age 56);
- have not yet attained age 65; and
- are still working.
If you have retired or have reached age 65 and are still working, use the Application for Account-Based Pension form.
Use this form to change the amount of your pension payments or the frequency, including a change of payment month for quarterly and annual payments.
